The Wrap-around Supports Stream is focused on collaboration across social services needed to improve health equity and address the social determinants of health affecting all members of the community. The priorities of the Wrap-around Supports Stream surround the suite of services required to provide a continuum of care that is tailored to a person’s needs in an organized and integrated manner including housing, basics needs, social services, and decreasing barriers to accessing services. The goal of the Wrap-around Supports Stream is a community where everyone can easily access housing and basic needs supports through clear, connected service pathways. We envision safe, welcoming environments that provide wrap‑around supports and promote dignity, autonomy, stability, and improved health.
